• Access to our services

    There are two ways you can access our services:

    1. You can contact Change Clinic directly to make a self-referral by either filling out the online form or calling 416-944-9292.  Our confidential administration office will ask you to briefly outline your reasons for wanting to use our service to ensure that we can help and arrange an initial appointment with you.
    2. You can be also be referred to Change Clinic or one of our therapists by your family physician, psychiatrist, or other relevant health professional.  Simply ask your referrer to send a letter, fax or email to us, including your contact details and a brief outline of your circumstances. A referral form can be found here
      Once we have your referral, our office will contact you within 24 hours to arrange your initial appointment.  If you are waiting for a professional to send us a referral, please feel free to contact us yourself to speed the process along.

  • Parking:
    Municipal parking ("Green P") is available off Bedford Road
    (one traffic light west of Avenue Road).  Metered street parking is available along Bloor Street and most side streets.

    Subway Stations:
    A short walk from either the Museum station on the University line or St. George station (Bedford exit) or Bay station (Yorkville exit) on the Bloor-Danforth line.
